When you call 866-829-8387, you will:
- Speak directly with a Transfer Center registered nurse. The nurse will contact the appropriate physician on call and facilitate a recorded consultation among the referring physician, accepting physician and the Transfer Center.
- Get the patient accepted for transfer or direct admission to the appropriate facility. Once the needs of the patient are determined, the patient will be accepted and transferred promptly for care.
- Receive communication about your patient. When a transferred patient arrives, the nursing supervisor will confirm with the Transfer Center. That same day, the Transfer Center nurse will send notification of the patient's admission by fax to the offices of both referring and primary care physicians.

Transfer Process Checklist
Below is the checklist of easy steps for transferring a patient to Mountain Vista Medical Center, Tempe St. Luke's Hospital or St. Luke's Medical Center:
- Call the Patient Transfer Center at 866-829-8387.
- Provide the patient's demographic information and clinical reason for transfer. You may be asked for additional information such as the patient's diagnosis, required level of care, current IV infusions, weight and any special equipment that will be required for transport - for example, an intra-aortic balloon pump, ventilator, external/internal pacer, etc. The Patient Transfer Center will make all necessary air or ground transportation arragements, if needed. Once the transfer arrangements are finalized, we will provide an estimated time of arrival.
- Obtain transfer consent by having the patient or an authorized family member sign an EMTALA-compliant transfer form.
- Complete a physician's certification statement of medical necessity for the patient's transport (Medicare patients only).
- Prepare a copy of the patient's chart.
- Prepare copies of all pertinent diagnostic studies related to reason for transfer - for example, lab work, EKGs, etc.
- Fax a demographic sheet to 480-358-6362.
- Send a confirmation of transfer to the referring physician/facility.
