IASIS Professional Liability Insurance Documents:

 There are two forms below. Providers are directed to the second form. 

1.     The first form is a billing form and is to be completed by the Practice Administrator for each provider requesting enrollment on the Harpeth Captive Insurance Product. It is located here: IASIS PL Coverage Billing Form.

2.     The second from is the application for enrollment on the captive carrier and is to be completed by the mid-level or physician provider who is requesting coverage.  It is located here: IASIS PL Application. Instructions on creating a digital signature are located here:  Application Instructional Video (.wmv format).
