- IASIS Healthcare - Catch-up Contribution Form
If you will be age 50 or older at any time during 2009, you can make up to an additional $5,000 pre-tax catch-up contribution during 2009 to your 401(k) Plan account if you meet one of the following plan limits:
- You contribute 50% (the maximum Plan contribution rate limit) of eligible Compensation to the 401(k) Plan for the entire year or
- You contribute $16,500 (the maximum dollar contribution limit) to
the 401(k) Plan in 2009.
You are not required to reach one of the above plan limits before you begin making catch-up contributions, but you must be contributing at a rate that allow you to reach one of the limits in calendar year 2009.
